Rehabilitation Services


Details of Service

Rehabilitation Services cater for people who have had a neurological event such as a stroke or new diagnoses of Multiple Sclerosis/Parkinson Disease or significant medical episode requiring prolonged hospital admission or following surgery. Services are developed to assist patients reach their functional goals through a multidisciplinary approach. We also offer group programs aiming to improve function for these people.

Cardiac Rehabilitation, Pulmonary Rehabilitation, Diabetes Exercise Programs and Cancer Survivorship Exercise Programs are also available:

  • Allied Health Assistant
  • Exercise Physiology
  • Occupational Therapy
  • Physiotherapy
  • Social Work
  • Speech Pathology
  • Group based interventions

Triage of Referrals

Triage will occur based on the clinical information provided on the referral.

Referrals are required for all group interventions.

Current Session Times and Locations

Locations: Melton Health and Community Services (Green Building)

Hours of Operation: Monday – Friday 8:30am – 4:30pm (variable depended on service being requested)

Important Information

Rehabilitation services are centre based. If home based therapy is required this will be completed through our Community Health Programs.

Not all services are 5 days a week.

Patients are not required to go through My Aged Care for referrals.

Group exercise based interventions such as Cardiac Rehabilitation, Pulmonary Rehabilitation, Diabetes Exercises and Cancer Survivorship must have a referral from a General Practitioner or Allied Health / Nursing clinicians.

Group based interventions must be goal based with aim for progression into the Community Health programs as appropriate.
