Service Description	Services include: Comprehensive education for the newly diagnosed, including group and individual education Education to clients, tailored to their specific needs Education for women with gestational diabetes Instruction in use of blood glucose meter, diabetes specific medications including injectables, and insulin pump therapy Assistance with insulin adjustment Provision of diabetes literature and videos Diabetes Support Group
Eligibility	People with diabetes or impaired glucose condition or family members of people with diabetes.
Access hours	Monday - Friday - 9.00am – 5.00pm
Referral process	Referrals are accepted from General Practitioners, Medical Specialists or other health professionals and can be faxed to our Referral and Appointment Service on 9746 0668. Clients can self-refer.
Cost to client	Pension or Health Care Card holders pay a reduced fee. Clients who do not hold a pension or Health Care card need to complete an income self-declaration form to determine fee payable and are charged in line with the Department of Health Primary Health Programs Fee Policy.
